Abstract

Position paper proposing a bridge specification between personal health platforms and hospital care systems. Extends the Five Model (Magoulas and Pessi) with data, knowledge, and context dimensions; applies Mintzberg professional bureaucracy to surgical micro-decisions; maps the gap across all six EU Interoperability Framework layers; and uses Ward and Daniel benefits management to measure impact as the delta from as-is to to-be. Introduces body as a boundary-spanning object (Star and Griesemer) where surgeon, patient, and AI share the same referent through different lenses.
